YOUTH APPLICATION
In order for your child to officially join BSA, he must have a completed youth application submitted to an adult leader at Troop 1 for processing. If you have not turned in an application, you may download one below and bring it to the next weekly meeting. |
ADULT APPLICATION
In order for you to participate in scouting events and activities, you must also fill out an adult application and submit it for processing. You may download one below if you have not done so and bring the completed form to the next weekly meeting. |

MEDICAL FORM
You will need to provide an updated Medical Form annually for your scout to participate in all outdoor scouting programs. Please make sure all the information in Part A, B, and C are complete before it is returned. You can download the medical form below. Part C is the only page that needs to be filled out by a physician. If the scout is going to Summer Camp, he will so need to provide the Drug Administration Record (as well as an Allergy Emergency Action Plan if he has/needs one).
